Cancellation Policy
All appointments will be booked with payment handled seemlessly through the Jane App. Payment for service is due upon booking your appointment with Jane. This secures your appointment booking, ensures payment and verifies your appointment with us.
If you need to cancel before our 24 hour cancellation policy, please complete this task via Jane. If you cancel less than 24 hours prior to your scheduled appointment 50% of the appointment fee is charged. If there is less than 12 hours up to a No Show for your appointment, 100% of the appointment fee is charged. No Exceptions to this policy will be entertained. Thank you.
Refund Policy
Refunds— as per our cancellation policy, if you cancel before the 24 hour period, Jane will remove the charge from your account and the appointment will be open and available to another patient to book in.
If the appointment is cancelled greater than 12 hours, but less than 24 hours, 50% will be charged to your method of payment. Thriving Health is working towards less administrative tasks for the office staff and the clinic owner. The clinic will be using Jane as much as possible to interface with bookings and payment.
Appointment Requirements
All appointments must have a concern(s)/symptom(s)/condition(s) attached or provided upon booking in order to give the necessary time needed to thoroughly address your needs. The Medical Office Assistant or Jane cannot book an appointment without known concern(s)/ symptom(s) or condition(s), therefore, stating an ambiguous reason (i.e., it is private/ I do not want to say) will not be accepted. Thank you for your understanding.
Introductory vs. Episodic Appointments
If a patient is new to Thriving Health, and desires care that is continuous/ongoing, an introductory appointment is required. After meeting, and both participants agree to the healthcare relationship, then the NP becomes your Family Nurse Practitioner. This step is vital to ensuring comfort, understanding, rapport, and alignment with personality, values and beliefs.
The Introductory Appointment is not a medical appointment, therefore, no care will be provided. The Introductory Appointment is a no charge 15 minute meeting. We ensure that any personal information disclosed during this meeting complies with all privacy legislation.
The patient will be disclosed of the reasons if the NP decides not to establish the NP-Patient Relationship, unless the NP believes that the disclosure will harm the patient, another person or pose a public safety threat.
An episodic appointment (same day/ walk in) is intended to address the patient’s presenting concerns, or identified medical conditions, and any follow up care as needed regarding those concerns.
The limitations and extent of follow-up will be explained to the patient during the appointment as episodic care is incidental, not continuous in its design. Follow up beyond the necessary care for the episodic appointment, is then arranged and referred back to the patient’s family MD or NP who has formally agreed to assume that responsibility.
If the patient does not have a responsible medical provider (i.e., family NP or MD) and the patient desires the NP at Thriving Health as their Family NP; it is left to the discretion of the NP to decide if an introductory appointment is needed and the patient(s) will be informed of this decision and an introductory appointment will be booked.
Ultimately, it is the NPs responsibility to ensure the patient is safe and cared for, and that is the intention of Thriving Health.
Thriving Health will never deny patient care based on complexity of condition (unless the NP believes his/ her current level of competency, skill, knowledge, experience, education is unable to provide proficient and safe care for that condition).
The NP cannot refuse to establish the NP-Patient Relationship based on discriminatory grounds, i.e., age, sex, marital status, medical complexity, physical or mental disability, national or ethnic origin, religion, race, sexual orientation and/or socioeconomic status.
Privacy Disclaimer
All information shared at Thriving Health NP Care Clinic is confidential and complies with provincial privacy legislation. During the introductory appointment, any information disclosed will be documented and stored securely. If the NP decides not to proceed with a patient relationship, an explanation will be provided unless doing so poses a risk to the patient or others.
